What this is
A real-time pelvic floor imaging assessment using Point-of-Care Ultrasound to see how your pelvic floor muscles move and coordinate with your breath and abdominal muscles. We can observe how your muscles contract, relax, and respond to changes in pressure, including in functional positions like standing.
We can assess symptoms of pelvic organ prolapse and identify structural support changes that may be contributing to pelvic organ prolapse or urinary incontinence. We can also assess bladder volume and voiding to better understand patterns related to urinary retention, urgency, and frequency.
What to expect
A small ultrasound probe is placed on the abdomen or perineum. You'll be guided through simple movements like contracting, relaxing, breathing, and bearing down while watching your muscles work on screen.

What this is
A surface electromyography assessment that measures the electrical activity of your pelvic floor muscles using external or internal sensors. It shows us how your muscles contract, relax, and coordinate, allowing us to identify patterns in muscle activity and tone.
This includes whether your muscles can fully relax when they should and contract when needed, which is often a factor in pelvic floor dysfunction.
What to expect
Sensors are placed externally or internally depending on your needs. You'll perform guided contractions and relaxation while the system displays your pelvic floor muscles' electrical activity in real time.

What this is
A detailed assessment of how the pelvic floor muscles supporting the rectum and anal sphincters function. It uses a single-use small, flexible catheter to measure pressure along the length of the anal canal and rectum.
The system generates a high resolution, color coded topographic map where different colors represent pressure levels. This creates a visual "pressure landscape" of muscle activity at rest, during contraction, with coughing, and with bearing down — helping us assess how effectively your muscles contract, relax, and coordinate during functional activities like a bowel movement.
What to expect
A thin, small balloon catheter is gently inserted into the rectum. You'll be guided through simple tasks such as squeezing, relaxing, coughing, and bearing down as if having a bowel movement. The system records squeeze pressure changes, strength, endurance, and ability to relax your muscles in real-time, displaying them as a color map to show exactly where pressures and tone are too high, too low, or not coordinating well.

This session is focused on progressing your treatment plan based on your symptoms, goals, and response to care.
Treatment may include:
- Manual therapy to address muscle tension, restrictions, or pain
- Breathing and pressure management strategies
- Guided exercises to improve strength, coordination, and control
- Movement retraining for daily activities and exercise
- Education and resource building to support your progress at home
- Sexuality counseling, when appropriate, to address sex education, pain, sexual function, and communication skills
What this is
A guided training session designed to retrain bowel coordination, rectal sensitivity, and pelvic floor muscle function using real-time balloon feedback. This includes both relaxation training for bowel emptying and strengthening strategies when needed for fecal incontinence.
What to expect
A thin, small balloon catheter is used to simulate the sensation of a bowel movement while you are coached through relaxation and bearing down strategies. This helps retrain how your pelvic floor muscles respond during a bowel movement and can also be used to improve strength and control if you are experiencing fecal incontinence.
Training may be performed on a treatment table or on a portable commode to better simulate functional positioning and practice.

Please be aware of our Telehealth out-of-state administrative fees. In light of supplementary obligations and resources essential for accommodating out-of-state patients seeking to initiate Telehealth care with our practice, an additional fee of $245.00 may be incurred for the evaluation. This covers the relevant legal procedures required on behalf of the practice. -
The Pelvic Health Evaluation comprises a comprehensive review of your health history, an examination, and the development of a detailed care plan designed to address your pelvic and sexual health concerns.
During a 90 minute evaluation, your pelvic therapist will:— Take ample time to listen to your story, understand your concerns, and address any questions or apprehensions you may have.
— Conduct a thorough pelvic health exam which could include checking the external genitalia, assessing the pelvic floor muscles, and possibly performing an internal rectal or vaginal exam.
— Rehabilitative pelvic floor ultrasound assessment to visualize your pelvic muscles and organs, improving pelvic floor awareness, coordination, and muscle control during therapy (Advanced testing includes additional fees).
— Discuss the assessment of any findings.
— Work collaboratively with you to set personalized goals and develop a comprehensive plan of care that addresses your specific needs.
Time outside of your session is also dedicated to detailed clinical documentation, care planning, and coordination. Following your appointment, you will receive a full evaluation report with findings and personalized recommendations to support your ongoing care and treatment goals.

Please note that a consultation doesn't establish a patient relationship and our pelvic therapists will not be able to diagnose or create a comprehensive treatment plan during a consultation.

An initial 90 minutes pelvic health evaluation is $489.
A 60 minutes single follow-up appointment is $184.
Sessions may be extended, depending upon the complexity of your health concerns. If a session extends beyond its scheduled time for longer than 15 minutes, an additional prorated fee, based on the session rate, will be incurred.
We value and believe in the power of an interdisciplinary approach. That’s why we offer discounted wellness packages to help you put your care plan into action. Wellness packages can be used with multiple providers across our multidisciplinary care team including: pelvic floor therapy, sexuality counseling, nutrition therapy, and mental health therapy. A shared decision making process with your pelvic health therapist is used to determine which combination of services are right for you.
Wellness packages may be used for both in-person and online services. Enrollment in wellness packages are available after an initial pelvic health evaluation or 30 minutes consultation. Wellness packages are non-transferable and non-refundable. They expire one year after purchase.

Our clinic does not follow protocols or set a generic number of sessions. Your health blueprint is unique and we believe in an individualized care plan based on your specific needs, goals, and resources.
After your initial evaluation, you and your pelvic health therapist will discuss an approximate number of sessions that are informed by the complexity of your health concern. -
Our clinic is out-of-network. This means that we are not contracted with any insurance companies including Medicare and Medicaid. Out-of-network does not mean no coverage. Every insurance company’s out-of-network policy is different and you’ll need to check with your insurance company to determine your out-of-network benefits and reimbursement requirements. Third-party companies, like Reimbursify, can make this process painless.
We offer appointment packages priced as low as $166 per appointment. After your initial pelvic health evaluation or consultation, you’ll have the opportunity to sign up for a package that fits your financial needs. Packages can be used with all providers across our interdisciplinary care team for both in-person and online appointments.

You do not need to see your Primary Care Provider to be referred for pelvic floor therapy. North Carolina is a direct access state for physical and occupational therapy services.
